Replacement Form

Complete the form below to request replacement items. Please only submit one form per item that needs to be replaced. Once your form is submitted, Bomb Party will review the details and start processing your product replacement. The processing time for replacement items is currently seven (7) business days.

Please note that if an item is marked out of stock, you may still submit a request with this replacement form. After our review, we will notify you that a similar item, or an item of similar purchase price, will be shipped to you.

Order Number

Enter the number for the order that contains the item you are hoping to replace. If you are a Rep and have already replaced a customer’s item, please enter the inventory order number containing the item in question.

Rep ID

If you are a Rep, this is where you enter your personal Rep ID.

Full Name

Please use the name of the customer or Rep to whom we will ship the replacement.

This must match the address on the original order. This may be a street address or PO Box.
